School/Sports Physicals
Board-certified nurse practitioner will complete a school/sports physical for patients 6yo and up! This is an in-person visit as it provides greater accuracy to the physical assessment.
Medication Refills
This visit is perfect for clients with an established primary care provider/physician (PCP) or specialist. If you anticipate running out of medication prior to your next appointment and are in need of a short-term refill (30-day supply or less) book today! Please have your prescription bottle and most recent lab values (if available) ready as we will review and discuss these items during the appointment. As a reminder, follow up regularly with your primary care physician. Report to the nearest urgent care/emergency department if symptoms worsen.
Sick Visit
Feeling under the weather? Discuss present symptoms and treatment options. If applicable, medications will be called in to the local pharmacy of your choosing. Remember, sick notes are not just intended for those with physical aliments, mental health days are just as important! Always follow-up with your PCP, the emergency room, or urgent care near you if symptoms worsen or do not resolve!
Sexually Transmitted Infection (STI) Testing
Keeping up with your sexual health is imperative! Meet with your provider to specifically discuss past sexual history, current symptoms, and receive lab orders. Included in this service is diagnosis, treatment, and/or referral (if indicated).
Disabled Person Parking Permit
Evaluation of temporary (6 months or less) for a Florida Disabled Person Parking Permit. Form will be completed and sent to you should you meet Florida Statue requirements.
